Description
Reporting to the Director of Health & Wellness, the Health Care Aide provides personal care and performs selected tasks, which are within their knowledge and skill, under the direction of their supervisor while adhering to regulatory and Revera's standards, policies and procedures.
Key Responsibilities
In conjunction with the Occupational Therapist, develops implements and evaluates individualized therapy programs.
Co-ordinates current stretch and strut ambulation program and physio/exercise classes, as outlined on the monthly OT calendar.
Provides daily therapy as instructed on the absence of Occupational Therapist.
Provides ongoing evaluation of resident status.
Collaborates with clinical team to initiate referrals.
Instruct staff to ongoing care requirements for residents related to therapy programs
Develops and maintains up-to-date individual resident care plans through interdisciplinary care conference and care planning.
Maintains accurate and complete clinical records on all residents according to established documentation policies and procedures and ensure confidentiality of these records.
Assists in maintaining a physical and psychosocial environment which meets the needs of the residents.
Promotes a positive team spirit within the department and the home
Attends in-service education in order to keep informed of current trends in nursing practice.
Participates in the home’s learning needs assessment.
Follows directives/memos/policies within the Home.
Is aware of and promotes the Resident Bill of Rights and incorporates it in daily practice.
Demonstrates a sense of social and moral responsibility and upholds a just and fair code of business conduct in accordance with the Code of Conduct of Revera.. Behaves in a manner consistent with the values of the Company.
Performs other duties as assigned.
Qualifications
Must be a graduate of an approved Physiotherapy/Occupational Therapy Assistant Certificate program from a recognized Community College.
Current experience in a long-term care setting.
Oral and written fluency in English.
Proficiency in a second language consistent with the Resident population would be considered an asset.
Must have demonstrated ability with comprehensive knowledge of Physio/OT practices and techniques as they relate to Long Term Care Resident at various levels of cognitive functioning.
Willingness to pursue further education in relevant specialties.
